I understand that EngageTLH involves assisting community agencies in the Tallahassee area. The volunteer work may be physically or emotionally demanding. I affirm that my health is good and that I am not under the care of a physician for any undisclosed condition that might endanger my own health or the health of other participants. I further recognize the risk of physical injury while participating in this program. I therefore release, discharge, and hold harmless Florida State University and the Center for Leadership & Service from any and all liability for injury to me from participation in, as well as any transportation provided, for the EngageTLH program.
I acknowledge the fact that there may be pictures or video taken during our service. Therefore, by initialing the box provided, I authorize the Center for Leadership & Service to use my photograph(s) for the specific purpose of publications and promotion and hereby release and hold harmless the Center for Leadership & Service from any liability. If I elect not to initial the box provided, it is assumed that I do not wish to have my photograph taken, and therefore it is my sole responsibility to be aware of my surroundings and to move away from any cameras or video equipment to avoid having my picture taken.
